I do hereby solemnly affirm and declare on oath as under:

    1. Compliance with Rules: I will strictly abide by all the rules, regulations, policies, and code of conduct enforced by the University now and those framed thereafter during my stay at the institution.
    2. Academic Integrity: All the educational documents, certificates, and transcripts submitted by me for the purpose of admission are genuine and authentic. If any document is found fake or forged at any stage, the University reserves the right to cancel my admission immediately and take legal action.
    3. Discipline and Conduct: I will not indulge in any activities prejudicial to the discipline, peace, and academic atmosphere of the campus. I will not join any political, sectarian, or illegal student organizations.
    4. Attendance Requirement: I am fully aware of the mandatory attendance policy. I understand that if my attendance falls below the required threshold (typically 75%), I will not be allowed to sit in the final examinations.
    5. Initial Semester Fee Payment: I explicitly undertake and guarantee that I will deposit exactly 70% of my total semester fee on or before 15-07-2025.
    6. Training Discontinuation & Fee Refund Policy: In the event that I leave or discontinue my training program, the semester fee refund policy will strictly apply as follows:
      • Leaving within 30 days (on or before 31-10-2026): 75% of the fee is refundable.
      • Leaving within 3 months of commencement: 50% of the fee is refundable.
      • Leaving after the mid-term examinations: 0% (No refund) is admissible.
    7. Mandatory Enrolment Fee: I acknowledge that a mandatory Enrolment Fee amounting to Rs. 10,000/- must be paid in full by the deadline of 30-06-2026.
    8. Enrolment Fund Refund Policy: I understand and accept that the enrolment fund is refundable only if a formal claim is processed before 10-07-2026. After 10 July 2026, no refund claims shall be admissible under any circumstances.
    9. Remaining Fee Deferred Payment: I promise to clear all remaining balances of my outstanding university dues after September, strictly adhering to the approved monthly or installment schedule assigned by the finance department.
    10. Process-Linked Institutional Charges: I will clear all Registration fees, Examination fees, University shared fees, and the official Enrolment Fees for PNMC-AHC exactly at the time of process execution as demanded by the authorities.
    11. No Ragging: I will not engage in, support, or promote any form of ragging, physical violence, harassment, or bullying against any fellow student or staff member.
    12. University Rights: I accept that the decision of the University authorities regarding any disciplinary matter, fee default penalty, or academic disqualification will be final and binding upon me.
